Walk-In Evaluation Clinic

Are you or someone you care for having issues getting around the house? Feeling unstable, or maybe even tripped recently? HealthSouth may be able to help with a free assessment that will only take about 30 minutes. Rehabilitation is an important part of the recovery process from many different illnesses and chronic conditions. We can help return someone to a more independent lifestyle. We admit from acute hospitals, physician’s office, the emergency room or from the patient’s home.

The Assessment Process: The evaluation is free and is conducted by a HealthSouth representative in a private room. With the patient’s consent, medical history, medications, and current balance, walking ability, physical deficits, swallowing, pain, memory and orientation are recorded. The assessment will take about 30 minutes. The patients’ referring or primary care physician will be contacted for a referral prior to admission.

Patients will be assessed for either Inpatient or Outpatient Admission:
- Inpatient Admissions must be medically stable, but have medical and rehabilitation needs; and
have the need for two disciplines of therapy: Physical, Occupational, or Speech.
- Outpatient Admissions must be medically stable and have the need for at least discipline of therapy: Physical, Occupational or Speech.

Most Common Candidates are individuals experiencing difficulty with:
Amputation
Active Polyarticular Rheumatoid Arthritis
Back Injuries
Balance disorders
Brain Injuries/Brain Tumor
Burns
Cardiac disorders
Multiple sclerosis
Neurological disorders
Neurological Disorders, including, but not limited too:
- Multiple Sclerosis
- Encephalopathy
- Guillan Barre
- ALS
- Parkinson’s Disease
- Late Effects Polio
- Friedreich’s Ataxia
Myopathy and Neuropathy:
- Critical Illness Myopathy
- Toxic Myopathy
- Uremic Myopathy
- Disuse Myopathy
- Peripheral Myopathy
- Polyneuropathy
Orthopedic Injuries
Pain – acute or chronic
Parkinson’s disease
Stroke
Traumatic Injuries
